Billing and Insurance

The following information is provided as a guideline.  If you have a specific question or concern
that is not answered here, please contact our office.



Orthopedic Surgery and Sports Medicine Group provides care for several HMO’s, PPO’s, 
IPA’s and other insurance and governmental programs.  Please check with your insurance
company to verify our participation with your group.

If you are insured, please bring your insurance identification card with you for each visit and
we will bill your insurance for you. If you have a PPO or HMO insurance, your co-payment
and any deductible will be due at the time of service.

We appreciate your payment at the time of service. For your convenience we
accept VISA, MasterCard, personal checks or cash.
